Any personally identifiable information that you submit may be saved in a database, and may be aggregated to generate non-personally identifiable information (i.e., information that cannot be attributed to any given individual). For example, a demographic profile of visitors to the Sites may be generated from the database. Non-personally identifiable information may be used by the Donor Network for any purpose. You will not receive any communications from us. Personally identifiable information will not be disclosed to any individual or business not affiliated with the Donor Network, except that (1) personally identifiable information may be disclosed when the disclosing party believes in good faith that law or regulation requires such disclosure; and (2) third party contractors may be provided with limited access to personally identifiable information in the course of operating and maintaining the Sites. Access to personally identifiable information by such contractors shall be limited to the information reasonably necessary for them to operate and maintain the Sites. The Donor Network shall obtain agreements from all such contractors to comply with the provisions of this Privacy Policy. We may track your IP address and your browsing activities (including but not limited to traffic patterns, click-stream patterns, search terms entered by users, and the URL of Sites that directed you to the site) but this information will not be associated with any of your personally identifiable information. It may, however, be aggregated to generate non-personally identifiable information, which we may use for any purpose.
Our website's optional log-in sign up forms may require users to select a user name and password. The forms also require that users give us contact information that may include name, e-mail address, format preference (HTML vs. Text), address, interests, and similar information. We do not request or store sensitive information from our visitors, such as credit card or social security numbers. Users who sign up have the option at any time of opting out and will receive no further communications from the Donor Network.
The New York Organ Donor Network invites website visitors to sign up to receive e-mail news. Initial information requested will include first name, last name and e-mail address. Because participation is optional, visitors may choose to submit their information or skip this step. If they do opt-in, they will be entitled to opt-out at any time. The Donor Network stores this information for the sole purpose of sending e-mail news. The Donor Network may send e-mails to subscribers seeking additional demographic and other information. Visitors will have the option of responding or not to such requests.
The New York Organ Donor Network's online surveys request information that includes: First and last names, e-mail address, and check-off boxes for questions such as whether the visitor would like to receive e-mails and/or postal mail from the New York Organ Donor Network. Because participation in online surveys is optional, visitors may choose to submit the surveys or skip this step.
If a user elects to use our referral service for informing a friend about one of our Sites, we ask for the friend's name and e-mail address. New York Organ Donor Network will automatically send the friend a one-time e-mail inviting them to visit the site. The Donor Network stores this information for the sole purpose of sending this one-time e-mail.
Signing up to receive e-mail news or logging in/signing anywhere on the site are entirely separate from the course of action that involves enrolling in the New York State Donate Life Registry, which is a confidential database administered by New York State's Department of Health. Visitors have the option of notifying the Donor Network that they have enrolled in the Donate Life Registry.
By enrolling in the New York State Donate Life Registry, individuals 18 years of age and over give legal consent for the recovery of their organs, tissues and eyes for the purposes of transplantation and/or research at the time of their death. Registry information is kept strictly confidential and can only be accessed by a) Department of Heath employees when required for the performance of their official duties, b) federally regulated organ procurement organizations, c) New York State licensed tissue and eye banks and d) other entities formally approved by the Commissioner. Such access can only be for the purpose of identifying potential organ and tissue donors at or near the time of death.
We collect an IP address from all visitors to our Sites. An IP address is a number that is automatically assigned to your computer when you use the Internet. We use IP addresses to help diagnose problems with our server, administer our Sites, analyze trends, track users' movement, gather broad demographic information for aggregate use in order for us to improve the site, and deliver customized, personalized content. IP addresses are not linked to personally identifiable information.
Our Sites may use cookies to enhance your experience while using our Sites. Cookies are pieces of information that some websites transfer to the computer that is browsing that website and are used for record-keeping purposes at many websites.
